RPM

Remote Patient Monitoring

Monitor patients' physiological data remotely using connected devices. Track blood pressure, glucose, weight, and oxygen levels for proactive chronic disease management.

What is Remote Patient Monitoring?

Remote Patient Monitoring (RPM) is a Medicare program that enables healthcare providers to monitor patients' physiological data outside of traditional clinical settings using connected medical devices. RPM allows practices to track vital signs like blood pressure, blood glucose, weight, and oxygen saturation in real-time, enabling proactive intervention before conditions worsen.

For practices managing patients with chronic conditions, RPM transforms care delivery from reactive to proactive. Instead of waiting for patients to report symptoms or present with complications, care teams can identify concerning trends early and intervene appropriately.

Clinical impact: Studies show that RPM programs can reduce hospital readmissions by up to 50% for conditions like heart failure and COPD, while significantly improving blood pressure and glucose control in hypertensive and diabetic patients.

RPM Devices We Support

CareStream provides and manages a complete ecosystem of FDA-cleared, cellular and Bluetooth-enabled monitoring devices:

Blood Pressure Monitors
Glucose Meters
Digital Scales
Pulse Oximeters
Thermometers
Heart Rate Monitors

Conditions Ideal for RPM

RPM is particularly effective for patients with conditions that require regular physiological monitoring:

  • Hypertension: Daily blood pressure monitoring with alerts for readings outside target ranges
  • Diabetes: Glucose monitoring with trend analysis and hypoglycemia/hyperglycemia alerts
  • Heart Failure: Weight monitoring to detect fluid retention and early signs of decompensation
  • COPD: Oxygen saturation and symptom monitoring for early exacerbation detection
  • Chronic Kidney Disease: Blood pressure and weight monitoring for volume management
  • Post-surgical recovery: Vital sign monitoring during recovery periods

How RPM Works

CareStream manages the entire RPM workflow from device deployment to clinical escalation:

01

Patient Identification

We analyze your EHR to identify patients who would benefit from RPM.

02

Device Deployment

Cellular-enabled devices shipped directly to patients with simple setup instructions.

03

Patient Training

Phone and SMS-based support to ensure patients can use devices correctly.

04

Data Collection

Automatic transmission of readings to our platform, with no patient action required.

05

Intelligent Monitoring

AI-powered analysis to identify concerning trends and outliers in the data.

06

Clinical Escalation

Immediate alerts for critical readings, queued reviews for non-urgent issues.

07

Documentation

Automatic documentation of all readings and interventions in compliant format.

08

Billing Support

Compliant claim generation when billing thresholds are met.

RPM Billing Codes

RPM services are billed using the following CPT codes:

CPT CodeDescription
CPT 99453Initial setup and patient education on device use (one-time)
CPT 99454Device supply and daily recording/transmission (monthly, requires 16+ days of data)
CPT 99457First 20 minutes of treatment management services per calendar month
CPT 99458Each additional 20 minutes of treatment management services

Stacking opportunity: RPM can be billed alongside CCM or APCM for the same patient, maximizing revenue while providing comprehensive chronic disease management.

RPM Program Requirements

To bill for RPM services, the following requirements must be met:

  • FDA-cleared devices: All monitoring devices must be FDA-cleared for the intended use
  • 16-day transmission requirement: Data must be transmitted for at least 16 days per month to bill 99454
  • Interactive communication: At least 20 minutes of interactive communication required for 99457
  • Chronic condition: Patient must have a chronic condition (acute conditions can qualify under some circumstances)
  • Patient consent: Written consent required before initiating services
